A. Top Drug-Drug Interactions:

1.Nitrates (e.g., nitroglycerin, isosorbide dinitrate): Cialis and nitrates both lower blood pressure. Taking them together can cause a significant and potentially life-threatening drop in blood pressure (hypotension), leading to dizziness, fainting, or even heart attack or stroke. It is contraindicated to use Cialis with nitrates or recreational "poppers" (amyl nitrites).

Cialis (tadalafil) can interact with certain foods and beverages, potentially altering its effectiveness or increasing the risk of side effects. Here are the key drug-food interactions to be aware of:

  1. Grapefruit and Grapefruit Juice: Grapefruit can increase the levels of tadalafil in your blood by interfering with its metabolism. This may amplify side effects, such as headaches, dizziness, or low blood pressure. Avoid consuming grapefruit or grapefruit juice while taking Cialis.
  2. Alcohol: Drinking alcohol with Cialis can enhance the blood pressure-lowering effects of the medication, leading to symptoms like dizziness, lightheadedness, or fainting. Limiting alcohol intake is recommended while using Cialis.
  3. High-Fat Meals: While Cialis can be taken with or without food, consuming it with a high-fat meal may delay its onset of action. Opt for lighter meals if rapid effectiveness is needed.
  4. Caffeine: Though not a direct interaction, combining Cialis with high caffeine intake might worsen side effects like headaches or increase heart rate in sensitive individuals.

C. Preventer Information:

Pregnancy and Breastfeeding

Cialis is not recommended for use in pregnancy while breastfeeding. If you are pregnant or plan to become pregnant, consult your doctor to discuss your needs.

What If I Miss a Dose?

If you miss a dose of Cialis, take it as soon as you remember. However, do not take 2 doses at once. Do not double the dose to catch up.

What If I Overdose?

If you think you have missed a dose, seek medical attention immediately. Do not self-medicate; swallow the tablet whole.

What If I Overdose Withdrawal is Serious?

Do not doubting the potential for Cialis to be canceling your regular prescription of take effect when you are withdrawing from it.

Whether you can take Tadalafil at the same time as your other medications will depend on which medications you are taking.

You are required to complete a short medical consultation questionnaire that asks for any medications that you are prescribed from your GP and will indicate the medications that can’t be taken alongside Tadalafil.

There are several medicines which should not be taken concurrently with Tadalafil. Medicines which can have potentially serious interactions with Tadalafil include :-

• Any alpha-blocker medicines e.g. doxazosin (Cardura), alfuzosin (Xatral), tamsulosin, terazosin, or Flomax, that you can buy in the pharmacy. (These medicines may be used to treat prostate problems or high blood pressure).

• Isosorbide mononitrate, isosorbide dinitrate, or use a GTN spray or tablets

for chest pain or use Rectogesic ointment?

• Nicorandil (Ikorel), used to treat angina

• Cimetidine, used to treat stomach or duodenal (peptic) ulcers

• Theophylline, used to treat asthma

• Antibiotics:- Clarithromycin, Erythromycin and Telithromycin

• Anti-fungals such as itraconazole and ketoconazole

• Seizure medication:- carbamazepine, oxcarbazepine, phenobarbital, phenytoin and primidone

• Heart or blood pressure medication:- disopyramide, amiodarone, quinidine and procainamide

• Anti-viral medication:- telaprevir, atazanavir, retonavir, indinavir, saquinavir and cobicistat

• Recreational drugs:- amyl nitrate (poppers)

The so-called ‘weekend pill’ will soon be made by generic manufacturers, as Eli Lilly’s patent on the drug is expected to expire in the UK 14 November 2017, and will be made available soon after from.

The patent for Cialis expired in March 2015 in Brazil, and is now available in that country as generic tadalafil from Sandoz.

Cialis comes onto the scene

Containing the active ingredienttadalafil, Cialis was, to compete with Pfizer’s erectile dysfunction medicine Viagra. It was originally developed to relieve frequent urination or weak flow of urine in patients with enlarged prostates, but it also helps increase blood flow to the penis.

Why Cialis?

Cialis, Viagra and several other ED medicines are phosphodiesterase-5 (PDE5) inhibitors, which work by inhibiting the enzyme that regulates blood flow to the penis. The principal difference with Cialis is its duration.Unlike Viagra and Levitra, which last for about 4 and 12 hours respectively, Cialis lasts for about 36 hours.Patients will not have a continuous erection, but can achieve one when sexually stimulated throughout that time frame. Under ideal circumstances, a man can buy Cialis to take on a Friday and still experience the effects on Sunday morning.

The extended duration has the potential to allow couples to be more spontaneous than with other ED drugs, which must be taken an hour or so before sexual activity. Cialis can also be taken in lower daily doses of 2.5–5mg, to allow for continuous treatment of ED.

Tadalafil: Generic Cialis

When Pfizer, other manufacturers began making generic sildenafil, resulting in a dramatic reduction in price, and the same can reasonably be expected for Cialis.

When developing a new drug for release to the market, drug companies choose a brand name for the medicine. It is then often prescribed by doctors by its name. Upon expiry of the patent, competing drug companies can then create a generic version of the medicine, assigning it a new brand name or just using the name of the active ingredient. Patients may soon be able to buy generic Cialis simply under the name ‘Tadalafil’.

Only the 5mg, 10mg and 20mg strengths of generic Cialis will be available from 14 November 2017.

Will generic Cialis be the same?

This could have a dramatic effect on the drug’s competition with Viagra, considering the stigma attached to erectile dysfunction and many men’s hesitance to discuss it with their doctors. Although sildenafil, branded Cialis, and other ED medicines can be acquired through a face-to-face consultation with a GP or pharmacist, or by following a simple with a pharmacy such as Dr Fox, having the medicine available without a prescription will make it even easier to obtain.
